Workstream / Activity

What is this work area about?

ReSPECT is a process that creates personalised recommendations for clinical care in emergency situations in which patients are not able to decide for themselves or communicate their wishes.

This workstream is about implementing and embedding ReSPECT within health and social care across the High Weald Lewes Havens (HWLH) and Brighton & Hove (B&H) localities.

Is the role meeting based?

Largely yes – through the ReSPECT Implementation Working Group (plus possible attendance at patient and public engagement events and other fora – see below)

Can there be virtual contribution (e.g. commenting on plans)? 

Yes, if the lay member is unable to attend a particular Implementation Working Group meeting in person by e.g. commenting on patient engagement & communication plans. Skype attendance might also be available in these circumstances.

Role of lay members

  • To attend regular ReSPECT Implementation Working Group meetings and :
    • contribute to discussions from a patient / carer perspective.
    • make sure that the patient and their family / carer’s voice is considered throughout the planning and implementation processes; to make sure the group thinks about the effect of proposals on patients, their families and carers.
  • To engage with other patient groups to feed in planning for the ReSPECT implementation (and vice versa).
  • To possibly attend other CCG patient & public engagement events (e.g. Big Conversation).

Background/ skills/ knowledge required

  • Comfortable working with a subject matter that involves End of Life and Palliative care discussions
  • Comfortable working with a group which comprises senior clinicians and senior commissioning managers
  • Knowledge of local area and local resident population (HWLH and / or B&H)
  • Comfortable attending / representing CCGs at public / patients forums and groups and collating feedback for the Implementing Working Group where appropriate.

Membership/format of group

Who is on the group, or involved with the work?

The ReSPECT Implementation Working Group includes:

  • CCG GP Clinical leads (for EOLC);
  • senior commissioners;
  • communication & engagement managers;
  • primary care workforce trainers; and
  • representatives from acute and secondary care providers (SCFT, BSUH) and from our local hospices.

Time commitment

  • This is a task and finish group which will be in place until ReSPECT has been rolled out and embedded within health and social care across HWLH and B&H.     It is currently envisaged that ReSPECT will be implemented, in a phased approach, by autumn 2020.
  • The Implementation Working Group will meet (approx.) monthly.
  • Possible attendance at engagement events and other fora, as outlined above.

Location

Implementation Working Group meetings will be largely held at HWLH CCG in Friars Walk, Lewes (Skype attendance might also be available, if required in some circumstances). 

Training /induction

A short induction/briefing will be offered by the ReSPECT programme lead 

Reward and reimbursement

Participants should be offered reimbursement of travel expenses and a recognition fee. (In BHCCG this is £25 per half day in M & S vouchers )

ReSPECT Implementation Working Group

Terms of Reference

  • Constitution of Working Group

The Working Group will be a task and finish group and will be in place until the ReSPECT document has been rolled out and is embedded within health and social care across the CSCA – South directorate localities.

  • Purpose of ReSPECT

The ReSPECT process creates personalised recommendations for patients’ clinical care in emergency situations in which they are not able to decide for themselves or communicate their wishes. The ReSPECT document is a summary plan that supports existing Advance Care Planning of future care wishes and details clinical discussions with patients and carers about Ceilings of Treatment that determine an individual’s Goal of Care.

  • Desired Outcome for the Group

A rolling process, which implements the ReSPECT document across health and social care across BH CCG and HWLH CCG by (around) autumn 2020. 

    • Settings

ReSPECT will be introduced across health and social care settings in BH CCG and HWLH CCG, including:

  • Primary Care
  • Acute providers
  • Community Hospitals
  • Ambulance service
  • Out of Hours service
  • Hospices
  • Residential Care Homes and Nursing Homes
  • Community settings, including people’s homes
    • Services

To implement ReSPECT across these settings, it will be necessary to involve:

  • National ReSPECT Team
  • Sussex ReSPECT Collaborative / wider STP
  • ESBT Implementation Steering Group
  • GP Practices within BH CCG and HWLH CCG
  • Acute hospital trusts – BSUH, ESHT, MTW
  • SCFT
  • SECAmb
  • Integrated Care 24 (IC24) out-of-hours service
  • BH Adult Social Care Home Forum
  • ESx Adult Social Care’s Care Homes Forum; care agencies; individual care homes
  • Hospices – serving the BH CCG & HWLH CCG area. 
  • Workstreams

To implement ReSPECT across these sectors and settings, the following workstreams will be developed.  Progress will be reported to the Implementation Working Group.

    • Adoption
  • Formal sign-off and adoption of ReSPECT by appropriate CSCA – BH CCG & HWLH CCG governance structures and committees.
    • Policy Revision
  • Embed ReSPECT in appropriate BH CCG & HWLH CCG Policies (e.g. HWLH CCG’s Vision for EOLC)
    • Communication and Dissemination
  • Develop a formal Communications Plan
  • Develop electronic means of sharing ReSPECT document
  • Communications with services and partner organisations (see 3.2 above)
  • Communications with patients and carers and general public
    • Training
  • Develop a formal training plan outlining the groups of staff that need training, how the training will be provided, and by whom.
  • Adapt mandatory training requirements, where appropriate
  • Deliver training to groups of staff outlined in the training plan
    • Implementation and roll-out
  • Develop detailed work-plans for a phased implementation and introduction across service providers and other partner organisations (see 3.2 above)
    • Monitoring and Review
  • Commission feedback from service providers and other partner organisations about ReSPECT and compliance levels
  • Commission patients / carers / families feedback
  • Evaluated feedback reported through CCGs governance structures and committees
  • Membership and attendance
